Manhattan District Attorney Cyrus Vance said Maksim Gelman, 23, had been charged with attempted murder, assault and criminal possession of a weapon for his alleged assault on a subway train on February 12.
Gelman pleaded not guilty, a spokesman for the prosecutor said.
The Ukrainian immigrant faces up to 57 years in prison if found guilty on all charges.
That's in addition to 175 years he faces if convicted in neighboring Brooklyn on four counts of murder, two of attempted murder, three assaults and four counts of robbery.
The slashing spree stunned a city that prides itself on having sharply reduced crime from the soaring levels of the 1980s.
In a more than 24-hour rampage, Gelman allegedly knifed to death his stepfather, ex-girlfriend and her mother, then killed a pedestrian while at the wheel of a stolen car, and then stabbed and wounded a string of other bystanders as he fled across the city.
Veteran police Commissioner Raymond Kelly said the spree was so horrendous and bizarre, we have no reason to know why he did this."
The latest indictment centered on Gelman's alleged attempt to kill a man on the subway under Times Square shortly before he was finally wrestled down and taken into custody.
"You're going to die!" Gelman allegedly told his victim before inflicting "serious physical injuries to the head, neck, and arm, which required medical treatment," the district attorney's office said.
